Brief Interview for Mental Status - Cognitive Assessment Tool

About BIMS

The Brief Interview for Mental Status (BIMS) is a validated cognitive assessment tool used in long-term care settings to evaluate cognitive function. It consists of three items: temporal orientation, recall, and pattern recognition.

1. Temporal Orientation (0-4 points)

Ask the resident: "What year is it right now?"

Ask: "What month are we in right now?"

Ask: "What day of the week are we in right now?"

2. Recall (0-4 points)

Say to the resident: "I am going to say three words for you to remember. Please repeat the words after I say them: 'sock, blue, bed.'"

3. Pattern Recognition (0-2 points)

Show the resident a picture and ask: "What do you see in this picture?"

BIMS Score Interpretation

  • 13-15: Cognitively Intact - No cognitive impairment
  • 8-12: Mild Cognitive Impairment - Some cognitive deficits
  • 0-7: Moderate to Severe Cognitive Impairment - Significant cognitive deficits
  • Note: BIMS is a screening tool and should not replace comprehensive cognitive assessment
