About Creatinine Clearance
Creatinine clearance estimates kidney function and helps stage chronic kidney disease (CKD).
CKD Stages by eGFR:
- Stage 1: ≥90 mL/min/1.73m²
- Stage 2: 60-89 mL/min/1.73m²
- Stage 3a: 45-59 mL/min/1.73m²
- Stage 3b: 30-44 mL/min/1.73m²
- Stage 4: 15-29 mL/min/1.73m²
- Stage 5: <15 mL/min/1.73m²
Formulas:
- Cockcroft-Gault:
- • [(140-age) × weight] / (72 × Cr)
- • Multiply by 0.85 for females
- MDRD:
- • 175 × (Cr)^-1.154 × (age)^-0.203
- • Multiply by 0.742 for females
- • Multiply by 1.212 for blacks
- CKD-EPI:
- • More complex, age/gender/race specific
Clinical Uses:
- • CKD staging
- • Drug dosing
- • Risk stratification
- • Treatment planning
Limitations:
- • Not accurate in acute kidney injury
- • Affected by muscle mass
- • May overestimate in elderly
- • Requires stable creatinine
Professional Resources:
- • Nephrologist
- • Primary care physician
- • Clinical pharmacist
